The South Dakota Army National Guard has a requirement for Soldier Medical & Dental Health Assessments.
The Medical & Dental assessments are required at multiple locations and dates within, SD. See Solicitation Medical Event Schedule for specifics.
The contractor is required to provide all management, labor, material, Medical & Dental Equipment and supplies for conducting exams onsite. Some events require examination space to be provided by the contractor. The contractor is required to be HIPPA qualified and enter assessment data (CACC Access) into MEDPROs & DENCLASS.
The request for quote and requirement details are listed within W912MM-17-T-0059 solicitation Performance Work Statement and attachments.
The Government will evaluate each prospective offer utilizing proposed costs for services at each location based on the estimated numbers provided. The contractor must be found acceptable based on the factors listed in Clause 52.212-2 in the Solicitation. The contractor will send in items listed in this clause.
No award shall be made unless the Contracting Officer makes confirmation of affirmative determination of responsibility for the potential contractor. Experience or knowledge completing Military Medical & Dental Exams is required.
Potential contractor must complete the System for Award Management (SAM) Registration process prior to award of contract. The applicable NAICS is 621399 for Miscellaneous Health Services.
