VA Connecticut Healthcare System Statement of Work
2019

Objective
This document highlights the technical specifications and services being requested by the Connecticut VAMC for consideration towards the purchase for a Fundus Camera. Vendors under this proposal shall provide all equipment, accessories, applicable training, and manuals.
Project Management / Equipment Implementation
Product delivery will be the responsibility of the Vendors working with the Connecticut VAMC project management POC. The Vendor will be responsible for coordinating and performing the delivery, implementation, and training for the devices.
Background/Justifications The CARDIOHELP System is the world s smallest portable heart-lung support system designed to treat and to transport patients needing extended respiratory and/orB circulatory support. It is ideal for use in intensive care units, cardiac catheter laboratories, operating rooms and emergency rooms.Limitations imposed by the Eye Clinic to purchase a new Fundus Camera/Clarus 500 which currently is running on Windows XP is no longer supported by IT. The upgraded is needed in order to connect to the eye clinic PACs system and Zeiss Forum. The system must have DICOM ability to interface with our software and must transmit the images to the patient records. A fundus camera is essentially a microscope which views the back surface of the eye (i.e. the retina) This allows clinical pictures to be taken so that the clinician can follow certain disease processes over time.B

Vendor will be required to provide two user manuals for the Fundus Camera/Clarus 500
Vendor will be required to provide two service manuals for the Fundus Camera/Clarus 500
Vendor will be required to provide an on-station in-service on the proper use and setup of the Fundus Camera/Clarus 500
Vendor will be required to provide the cleaning methods/guidelines for Fundus Camera/Clarus 500and other accessories.
All employees who will be utilizing Fundus Camera/Clarus 500, including Physicians/Optometrists, Technicians, Bio-Clinical Engineering, and Clinic staff members ("Trainees"), must be trained by an authorized representative from PROAIM Americas, LLC. prior to using the System.
No charge on a mutually agreeable date and time.
