Respite and Extended Services
The State is seeking proposals to contract with one (1) individual or organization to serve as the fiduciary of funds and administer a statewide respite and department approved extended services program for eligible families under the State Children and Youth with Special Health Care Needs (CYSHCN) Program. Services will be provided in accordance with program policy and based on the availability of funds. Data reporting requirements will be used for program planning and evaluation.
