A. GENERAL INFORMATION

Title of Project: Purchase of Ultima Series1400 LXP Freestanding Glassware washing machine

Background: Dr. Waxman s group uses various sized glassware daily in the research laboratories. Glassware must be washed after each use. This is done in a glassware washing machine. The current washer is over 20 years old and has major problems that include leaks that cannot be repaired. We would like to purchase a new glassware washer to replace the old one currently in use.

Scope of Work: The selected Vendor will inside deliver, remove existing washer and soft hook-up to include final leveling of unit, connection of washer s water inlet hoses and drain hoses to required utility connection points as specified in installation manual within the laboratory of Dr. Waxman. The vendor will train members of the Waxman lab in the use and care of the glassware washer.

Period of Performance: To be delivered & installed within 30 days after receipt of order

Type of Contract: Firm Fixed Price

Place of Performance: The Contractor shall support this effort at the VA CT Healthcare system 950 Campbell Ave., Bldg 34, West Haven, CT 06516.


B. GENERAL REQUIREMENTS

For a period of twelve (12) months, the contractor shall provide technical support to our users of the purchased items

C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES

Task 1: Deliver and install specified items
The contractor will open and identify all the items shipped against the packing lists. The contractor will assemble and verify full functioning of the items. The contractor will consult and work with designated members of Dr. Waxman s lab as to the final placement of the items into Dr. Waxman s lab.


Task 2: System checkout and refresher training
The contractor will verify the operation of the installed components and demonstrate the proper operation of the newly installed components to designated members of Dr. Waxman s lab.


Task 3: Provide total technical support
The contractor will provide phone, email, and web-based tech support.


D. INSPECTION AND ACCEPTANCE CRITERIA

Final inspection and acceptance of all work performed, reports and deliverables will occur at the place of delivery designated by the COR. Final approval and acceptance of documentation required herein shall be by letter of approval and acceptance by COR. The Contractor shall not construe any letter of acknowledgment of receipt of material as a waiver of review, or as an acknowledgment that the material is in conformance with this SOW.
