I.SUMMARY AND BACKGROUND

The Winnebago Comprehensive Healthcare System (WCHS) is currently accepting proposals for implementation of a project to repair HVAC deficiencies in pressurized rooms identified during inspection of the Twelve Clans Unity Hospital in Winnebago, NE.

The purpose of this request is to solicit proposals from various candidate organizations, conduct a fair and extensive evaluation based on criteria listed herein, and select the candidate who best represents the direction that WCHS wishes to go.

Winnebago Comprehensive Healthcare System is a tribally managed healthcare organization operated under compact with the US Federal Government based upon Title V of the Indian Self Determination and Education Assistance Act.

II.PROPOSAL GUIDELINES

This Request for Proposal represents the requirements for an open and competitive process. Proposals will be accepted until 4:30 PM on 17 August 2019. Any proposals received after this date and time will be returned to the sender. All proposals must be signed by an official agent or representative of the company submitting the proposal.

If the organization submitting a proposal must outsource or contract any work to meet the requirements contained herein, this must be clearly stated in the proposal. Additionally, all costs included in proposals must be all-inclusive to include any outsourced or contracted work. Any proposals which call for outsourcing or contracting work must include a name and description of the organizations being contracted.

All costs must be itemized to include an explanation of all fees and costs.

Contract terms and conditions will be negotiated upon selection of the winning bidder for this RFP. All contractual terms and conditions will be subject to review by WCHS legal department and will include scope, budget, schedule, and other necessary items pertaining to the project.


III.GENERAL REQUIREMENTS

All work taking place under this RFP must comply with the 2012 NFPA Life Safety Code, and the 2014 FGI Guidelines for Design and Construction of Hospitals and Outpatient Facilities, and other regulatory requirements as listed in References section.
The contractor will ensure the normal business operations are not affected with minimal interruptions during construction. Contractor must comply with all Federal, State, and Local regulations. The Contractor is advised to confer with Winnebago Tribe of Nebraska regarding any Tribal taxes/permit required in conjunction with this work.

Contractor will be required to secure an Infection Control Construction Permit from the hospital Infection Control Officer, and ensure the costs of those precautions are included in the award price. Following award of the Task Order, a preconstruction conference will be held at the Twelve Clans Unity Hospital, and the Hospital Safety Officer will issue any additional permit requirements for working within the hospital.

Contractor will provide red-line drawings to the WCHS Project Officer at the completion of task order.

The Contractor shall restore all areas disturbed by construction, both interior and exterior, to condition at least equal to that existing prior to construction. Contract shall take necessary precautions to minimize any dust, dirt, or tracking throughout the health center during the construction process, and shall be responsible for any required clean-up or damage related to the construction activity. The Contractor shall dispose of all excess construction materials and rubbish in an EPA-approved landfill.

Technical drawings are available for estimating and execution of the construction. The Contractor is encouraged to contact the Contracting Officer (CO), or designee, to arrange a visit to determine the nature and extent of the required construction. Submittals will be required for all tasks marked below with an (*).


IV.CONTRACTOR EXPERIENCE REQUIREMENTS

All organizations submitting a proposal must have completed previous projects in a Healthcare Setting within the last 3 years; includes experience with the Infection Control and Safety Risk Assessment Process and ICRA Levels 1-4 used for dust management in a healthcare setting.

Modify existing ductwork as noted below (columns 3 & 5)
Balance HVAC in each room in the Table 1 below; requirements are based on the room being in the "occupied" or "in-use" mode

VI.BASIS FOR PAYMENT

In accordance with Winnebago Comprehensive Healthcare System and Winnebago Tribe of Nebraska contracting procedures; the basis for payment of this work shall be a lump sum price for the completion of the proposed contracts in accordance with Summary of Work and accepted proposals, including all labor and equipment necessary for completion of the work. The period of performance will be 90 days from the notice to proceed. All items listed above shall be approved by submitting shop drawings for approval by WCHS prior to order or installation.
