Curry Health District is soliciting bids from experienced and qualified contractors to complete an ALTA/ACSM Land Title Survey. Proposals will be received by the District only until 5 p.m. PST, Tuesday, March 25. Curry Health District reserves the right to cancel the solicitation or reject any and all proposals. Any deviation from the specifications must be noted on the proposal. The proposal must be received on or before the date and time given above.

PROJECT NAME: Curry Health District - New Hospital
PROJECT DESCRIPTION:
Curry Health District's hospital is located in Gold Beach, Oregon. A proposed new hospital building will be one or multi-story with an approximate square of 50,000 square feet. Project is expected to demolish existing single story hospital building of approximately 23,438 square feet. Additionally, five buildings on District property nearby which total approximately 7,700 square feet may be demolished or moved off site. Maps are included. The District is attempting to purchase five adjacent properties. The survey will include the District's current property, plus the adjacent five properties.
OVERVIEW:
Curry Health District is seeking land-survey services to complete an ALTA/ACSM Land Title Survey in accordance with minimum standard requirements for ALTA/ACSM Land Title Survey and adopted by American Land Title Association and National Society of Professional Surveyors. In addition, surveyors shall also conduct their surveys in accordance with all applicable jurisdictional requirements and standards of practice. Where conflicts exist between the standards, the more stringent shall apply. Work to be performed by and under the direct supervision of a Professional Land Surveyor licensed in the State of Oregon.
OPTIONAL SURVEY RESPONSIBILITIES AND SPECIFICATIONS:
As specified in Table A.
ALTA/ACSM LAND TITLE SURVEY:
Perform ALTA/ACSM Land Title Survey for the subject properties which will include lot numbers as shown in the attached Assessor maps:
10100, 10200, 10300, 10400, 10500 - Existing homes not currently owned by the District. If acquired, these homes will be demolished.
9600, 9700, 9800, 9900, 10000, 11100 - Curry Health District property including demolition of the existing facility
SCHEDULE OF WORK:
Work shall be completed within fourteen (14) calendar days after execution of the Contract document. The successful proposer shall provide all equipment, instrumentation, transportation, supplies and personnel to perform the requested services. The awarded proposer must have the capacity to immediately proceed with work and accomplish it in a timely manner.
COMPENSATION:
The amount for your services shall be a lump sum fee payable in accordance with terms of the Contract document upon delivery of the required reports. The lump sum amount proposed by your firm for the ALTA/ACSM Land Title Survey based upon USDA or other Governmental requirements is $_____________________ (state your fee). If your firm desires to propose a phased payment of this fee, attach to this executed form a breakdown of the phases and the associated fees. Include Reimbursable Expenses in your fee quote.
KEY PERSONNEL LIST:
Attach to this executed form a list of key personnel that will perform work for the project with resumes, licenses, or certifications evidencing qualification to perform this work.
HOURLY RATE AND STANDARD RATES SCHEDULE:
If applicable, and for the purposes of additional work that may be added by change order to your firm's scope of services, attach to the executed document a schedule of hourly rates and standard rates (including travel) that your firm proposes for the duration of the project.
SCOPE OF WORK:

CONTRACT AWARD:
Contract will be awarded to the firm with the best combined score which will be a mix of qualifications, pricing and timing.
By signing this document your firm represents that it has fully reviewed this bid solicitation, has furnished all required data, and has clearly identified all proposed changes, deviations, and/or additions to the requirements of Curry Health District.
