REQUEST FOR PROPOSALS #2013-001
AUDIT SERVICES
Sealed proposals will be received by the Secretary of the Board of Directors for iGeneration Empowerment Academy of Riviera Beach, FL located at 3970 RCA Blvd. - #7000A, Palm Beach Gardens, FL 33410 for the provision of an annual audit of the School as required by Section 218.39, Florida Statutes (the "Work").
In submitting a response to this RFP, the Respondent shall be the person or legal entity who will be entering into the Agreement with the Schools. Respondent shall:
1)Provide a five (5) page written proposal to perform the Work (the "Written Proposal"), which includes the following:
a.The Respondent's legal name(s), headquarters address, local office address, state of incorporation, key firm contact names, and an email address through which the Schools may communicate with Respondentt
b.A detailed summary of experience and qualifications to perform the Work, which addresses the Criteria, as defined belowt and
c.A lump sum price proposal per year for complete performance of all the Work for each of the Charter Schools. This lump sum price will be applicable for five (5) years.
d.Disclosure of all business-related litigation involving Respondent, any contract defaults by Respondent, and any disbarments from eligibility for government contracts occurring, initiated and/or concluded within the last five (5) years, which shall include the parties involved, the reason for the litigation, default, or disbarment, the dates or occurrence, initiation and/or conclusion.
2)The Written Proposal shall be submitted as an email attachment and sent to secretary for the Board of Directors for the Schools, Thomas E. Douglas, [email protected].
3)The Written Proposal and email must be received by 4:00pm May 9, 2013.
The Work may also include the provision of audit services for one additional school substantially similar to the Charter Schools, which are expected to be approved and commence operations by August 2014. The lump sum price proposal for the Charter Schools shall be applicable to these additional schools also.
In evaluating the Proposals received in response to this RFP, the Schools will consider the ability and experience of Respondent's personnel, experience of Respondent, ability to furnish the required services, experience providing audits under Section 218.39, Florida Statutes, experience with charter schools, and familiarity with the rules of the Auditor General adopted pursuant to Section 11.45, Florida Statutes (the "Professional Criteria").
The Audit Committee for the Charter Schools is comprised of the members of the Boards of Directors for the Schools, and the Audit Committee shall evaluate the Written Proposals as follows:
Professional Criteria - 60%
Reliability and Reputation - 20%
Price Proposal - 20%
Statement of Offer - The Written Proposal must contain the following statement:
I, as principal or agent of (the "Respondent"), hereby agree and certify that this Written Proposal constitutes an offer to the Schools to perform the Work set forth in the RFP in accordance with industry standards. This offer shall remain open until May 30, 2013, or until the RFP is awarded, whichever occurs first. The Respondent will enter into a written agreement consistent with the terms and conditions of this RFP and industry standards, within fifteen (15) days,
