This is a Request for Information (RFI) announcement only. This is not a solicitation. The purpose of this RFI is to gain knowledge of current market conditions relative to NAICS 621492, Kidney Dialysis Centers with a size standard of $38.5 Million. The Department of Veterans Affairs (VA), Network Contracting Office 23 on behalf of the Sioux Falls VA Health Care System, Sioux Falls, South Dakota, is seeking to identify any vendors capable of providing Off-Site Per Procedure Chronic Dialysis Services. The anticipated contract will be a firm-fixed price contract for a one year base, plus four one-year options.

Scope: The Sioux Falls VA Healthcare System requires contractual Acute Dialysis services to provide services to eligible beneficiaries. Please reference the example schedule of services listed on page 2 for the exact requirement of this contract.

Respondents are not limited to SDVOSB and/or VOSB vendors, all sources are encouraged to respond. The VA is seeking to identify all potential sources. Responses to this Sources Sought must include the following information:

Company Name, Address, Contact Person Name, Phone Number, Fax Number, email address(es), DUNS number and company website.

Socio-Economic Status of business (example: SDVOSB, VOSB, etc.)


What medical services does your company primarily provide?

Are you, or have you previously provided Acute Dialysis services to any federal treatment facility? If so, please provide contract number(s) for the previous three (3) years.


Will your company enter into a joint venture or other relationship in order to provide this service?
Responses to this RFI market survey must be in writing and emailed to charles.morin@va.gov. No telephone responses will be accepted, however you may call Charles Morin at 651-293-3035 with questions regarding this RFI. Responses must be received no later than 3:00 pm Central, October 16, 2018. No solicitation documents are available at this time. This RFI is to acquire information only. Vendors interested in providing an offer will have the opportunity to respond to a Request for Proposal that shall be published separately at a later date.



SCHEDULE OF SERVICES

The Contractor shall provide Off-Site Chronic Hemodialysis and Peritoneal Dialysis (PD) Services in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the Sioux Falls VA Health Care System (VAHCS), 2501 W. 22nd Street, Sioux Falls SD 57105. The Contract will be effective for a base period of one (1) year and include four (4) one-year option periods.
Services to be provided at the contractor-owned facility (facility ownership exempt in community nursing home setting). Contract prices as negotiated herein shall be honored at all contractor-owned facilities. The Centers for Medicare & Medicaid Services, Current Procedural Terminology (CPT), shall be used to identify the procedures and services performed.
All-inclusive Rates. The contractor shall be reimbursed a single, fixed fee/per treatment, all-inclusive rate. Exceptions to the per treatment rate include:
Nephrologist services (this is billed on separate line item in schedule of items.
Pre-authorized non-routine drugs
Pre-authorized medically necessary laboratory testing and blood transfusion products/services.

NOTE:
These exceptions are reimbursed at the current Medicare rate as of the date of service and are not included under this contract. Services rendered will be paid outside of this contract by other means, i.e., fee basis.
Contractor is required to maintain medical justification in the VA beneficiary s community dialysis medical record for the separate billings of those services not included in the per treatment rate excluding nephrologists billings.
Contractor is required to submit medical justification (fax, mail) along with the required medical claim information for issuance of payment.

Comprehensive, all inclusive, Per Treatment Pricing . The Contractor will bill each episode of care on a per treatment price basis which is an all-inclusive rate to include all equipment, facility, nursing, technical personnel, professional services (social work, dietary, etc.), supplies, lab tests, and drugs that are necessary to furnish dialysis treatment in an outpatient setting. The need for any additional services must be documented by the Contractor in the patient s medical record and be reflected in the billed ICD9-CM diagnosis code. The Contractor will not be reimbursed for any test or medical services that are not identified in this contract without prior authorization by designated VA personnel.
Contractor must follow current Medicare guidelines for treatment. If Medicare changes their guidelines for treatment as well as additional services, the Contractor will adopt and implement such guidelines.

South Dakota: Campbell, McPherson, Brown, Walworth, Edmunds, Day, Potter, Faulk, Spink, Clark, Codington, Grant, Deuel, Hamlin, Spink, Sully, Hyde, Hand, Beadle, Kingsbury, Brookings, Hughes, Lyman, Buffalo, Jerauld, Sanborn, Miner, Lake, Moody, Brule, Aurora, Davison, Hanson, McCook, Minnehaha, Tripp, Gregory, Charles Mix, Douglas, Hutchinson, Turner, Lincoln, Bon Homme, Yankton, Clay, and Union.

Iowa: Lyon, Osceola, Dickinson, Emmet, Sioux, O Brien, Clay, Plymouth, Cherokee, Woodbury.

Minnesota: Lac Qui Parle, Yellow Medicine, Lincoln, Lyon, Pipestone, Murray, Cottonwood, Watonwan, Rock, Jackson, Martin, Nobles.

GEOGRPAHPHIC AREAS CONTRACTOR HAS A FACILITY OR SATELLITE CLINIC. It is the VA s intent that any contractor offering services under this contract will honor the pricing proposed at any of their locations that are located in the catchment area of the state and counties listed above. Offerors submitting a proposal must offer all CLINS in the state if they offer pricing for that state. Contractors pricing offered will be honored at all contractor-owned facilities in the state they proposed pricing on.

Patients currently receiving treatments at a facility that is not under a contract, will be converted to the contract rate upon contract award.
The contract will contain one (1) base year and include four (4) one-year option periods. The Contracting Officer shall determine (based on an integrated assessment of each offeror s technical and price proposal) the individual proposals evaluated to be most advantageous to the Government.
