Responses to this notice must be submitted in writing via email and must be received not later than Friday, June 28, 2019, 4:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). WORK DESCRIPTION
GENERAL INFORMATION

Title of Project: Replace the Expansion Joints deficiency located in building: 135, at the Lyons VA Medical Center, 151 Knollcroft Road, Lyons, N.J. 07939.
Scope of Work:
The Vendor shall provide all necessary materials and labor to replace expansion Joints in Bldg. 135,1c 9(8 X16 ), 2c (8 X14 ), 2b (8 X12 ) and 1b new wing (8 X6 ) ;(8 X4 ) ;(10 X6 )3 sections at the VA New Jersey Healthcare System 151 Knollcroft Road Lyons, New Jersey 07939.
The material required for the plates is Poly Epoxy. The manufacturer for our current joints does no longer exist.
The Vendor will install dust barriers and dust mat s ensuring that no dust interfere normal everyday operations.
The Vendor will mobilization and install containment where necessary. The containment is for the glue and other hazmat from the floor tile underlayment. Conditions under the existing flowing are unknown.
The Vendor will remove old expansion joint material and any Plates /encumbrances.
Vendor will clean and surface prep the existing Joints.
Vendor will reconstruct the joints as necessary utilizing 100% polymers.
Vendor will apply a 100% solids polymer material to bridge the expansion joints and provide a smooth transition for wheeled traffic.
Vendor will provide a temporary ramp over the expansion joints until cure for foot traffic.
Vendor will warranty all labor and materials for two years.
This Statement of Work is only for floor joints. It does not include walls or ceiling joints.
Most of the floor joints locations are covered with vinyl flooring so contractor can salvage this flooring and reuse them. Contractor must also replace the flooring that could not be salvaged.

Performance Period: A schedule will be set upon contract award by the COR, it is expected the work will be completed within 45 days.

GENERAL REQUIREMENTS
Contractor will repair the expansion joints for building # 7135
Contractor will have at least 5 years experience working on Polymers Expansion Joints.
Contractor will have a job Forman always overseeing the Expansion joint project.
Upon Completion ensure that all debris is cleaned up and removed from VA premises by contractor.
