Revise the Note 4. Due to timeframe, the schedule of the site visit is changed from "at 1000 hrs on 4 Jun 18" to "at 1330 hrs on 8 Jun 18."
The contractor shall submit Gate Pass Request Form with copies of valid ID with picture (Driver's License, Passport, or My Number Card, etc.), Vehicle Inspection Certificate, Voluntary Insurance for Automobile, and Compulsory Automobile Liability Insurance by 1000 hrs on 5 Jun 18.

Please come to the Parking Lot of the Camp Zama Main Gate at 1300 hrs on 8 Jun 18.
