Statement of Work
Abate Mold in B-400 AHU 1 & AHU 7(Admin Area)
Tomah VAMC Tomah, WI

On behalf of the Tomah VA Medical Center in Tomah, WI, the Great Lakes Acquisition Center is seeking notification from vendors interested in providing all necessary labor, materials, and tools necessary to Abate mold in Abate Mold in B-400 AHU 1 & AHU 7(Admin Area).

2.1 Contract or shall use negative pressure vacuum truck and provide any barriers necessary to contain all debris knocked loose.
2.2 Once cleaned the ductwork will be fogged with BBJ Mold Control for HVAC systems.
2.3 All work will be done on a standard weekend hours or weekday evenings (5:30 p.m.).
2.4 Description of the work and End Results Required; the vendor shall provide the following:

The end result is to Clean Equipment to eliminate a possible infection control issue

2.5 Quality Assurance:

2.5.1 Inspection/Acceptance; the Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance.

2.5.2 The Government may require re-performance of nonconforming services at no increase in contract price; the Government must exercise post-acceptance rights within a reasonable time after the nonconformity was discovered.


3. Hours of Operation
3.1 Contractor work will occur from as outlined above, unless otherwise mutually agreed upon to extend the hours of work.

3.2 Contractor to check in with the Fire Department located in building 36 when arriving on campus to pick-up badges and receive assess to the area needing to be cleaned

3.3 Period of Performance: Support is to occur as scheduled and at the request of the government and as required during the period of performance. Work shall occur only during weekend hours and would be expected to take no more than two weekends.


4. The contractor shall take all necessary precautions to protect government property and shall repair any damage caused by their efforts.


5. NAICS 562910 Remediation Services with small business size standard of $20.5 Million.


6. Anticipated price range: Less than $25,000.


Interested vendors shall provide confirmation of compliance with NAICS 562910 small business. Special consideration shall be provided to qualified CVE-Verified SDVSOB and VOSB vendors.

This item is a follow-on action to FBO Sources Sought VA69D-17-R-0758.
