Recruitment and Referrals
Product/Service Description: The Department seeks a contractor to work with the ten Community Health Centers in Vermont to coordinate the following activities: 1) implement a technical assistance and training component to recruit and refer smokers to free cessation treatment offered through the Vermont Quit Network; 2) provide incentives to Community Health Centers for implementing a plan, do, act, study process and successfully referring smokers to the Vermont Quit Network; 3) promote the Ladies First Program, a breast and cervical cancer and cardiovascular program to eligible women accessing the Community Health Centers network; and 4) provide incentives to Community Health Centers for enrollment, screening, and reporting of clinical results to the Ladies First Program. There are approximately 83,000 smokers in Vermont and 50,000 women eligible for Ladies First Program services.
