This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFP) K15-0002. This solicitation is issued on an unrestricted basis.
The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16.
The associated North American Industry Classification System code is 493110 and the small business size standard is $30 million.
PRICE/COST SCHEDULE - Cost for Receiving at Warehouse, Cost for warehousing, Cost for Delivery Cost Schedule
PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide receiving, warehousing, delivery, and installation of equipment and materials being ordered for a new Kayenta IHS hospital.
Date and Place of Delivery: Kayenta Indian Health Service Hospital, 1 mile East, Junction 160 & 163, Kayenta, AZ
STATEMENT OF WORK: The government requires secured storage space. Space must be indoors with forklift capability and climate controlled. Space must have adequate lighting. An alarmed security with fence is preferred with on-site 24/7 security. The space must be accessible to the government during anytime Monday - Friday and accessible on Saturday and Sunday upon request with 24 hour request notice. Space must contain dock space with adequate turn around for tractor trailers. See attached RFP for further information.

Copies of: (1) Certificate(s); (2) License; and (3) Insurance. PROVISIONS AND CLAUSES: The following provisions and clauses apply to this acquisition. The FAR provision 52.212-1 Instructions to Offerors Commercial Items; and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference.
CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.
PRO-CHILDREN ACT OF 1994: The Contractor certifies that it will comply with the provisions of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. PROPOSAL SUBMISSION INSTRUCTIONS:.All responsible offerors may submit a proposal, which shall be considered by the Agency. Offers shall be submitted to the Navajo Area Office, Department of Acquisition Management and Contracts, P.O. Box 9020, Window Rock, 86515 no later than 3:00 p.m., on December 29, 2014. The offer must be submitted in a sealed envelope, addressed to this office, showing the time specified for receipt, the solicitation number, and your company name and address.
