Combined Synopsis/Solicitation
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Fort Shafter, HI 96858, intends to solicit, negotiate, and award a Blanket Purchase Agreement (BPA) for Pathology supplies to include but not limited to Reagent Analyzers, Auxiliary Reagents, Hemostasis Analyzers, Screening Tests, Disposables and accessories.
This is a request for quotation (RFQ). This solicitation number is W81K02-17-T-0459. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The Standard Industrial Classification Code is 6515, size standard is 1000 employees and the NAICS code of 325413. This procurement is Unrestricted BRAND NAME OR EQUAL.
Provisions at 52.212-1, Instructions to Offerors - Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See W81K02-17-T-0459.
Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer.
The clause 52.212-4, Contract Terms and Conditions - Commercial Items, and its addendum applies to this acquisition. See W81K02-17-T-0459.
The deviation to clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. See W81K02-17-T-0459 for the FAR clauses that are applicable to this acquisition.
The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable.
The Governmente intends to award a Blanket Purchase Agreement.
Contract Requirements - The successful offeror shall be required to provide the Brand Name or Equal requirement for pathology supplies to include but not limited to Reagent Analyzers, Auxiliary Reagents, Hemostasis Analyzers, Screening Tests, Disposables and accessories that meet the salient characteristics given in solicitation W81K02-17-T-0459.
