The purpose of this RFP is to solicit proposals, select a contractor, and award a contract to provide an evaluation of the Improving Oral Health through School Based Services Program. Building Healthy Futures anticipates spending $25,000-30,000 for these services to be provided from February 15, 2014 through December 31, 2014, with the possibility for renewal in 2015-2016. The evaluation services will focus on the collective impact of the Oral Health program initiatives including the process implementation, outcomes (both physical and behavioral) as well cost effectiveness measures (i.e., return on investment).
