PROJECT DESCRIPTION
The Central Local Health Integration Network (CLHIN) has approved a full integration between Better Living Health and Community Services (Better Living) and Palliative Care Network for York Region (PalCare) effective January 1, 2017. Better Living is responsible for managing the subcontracts for five hospice organizations each providing Community Hospice Services and inclusive of all their additional programs and services within York Region and is accountable to the CLHIN for the delivery of services. The five hospice organizations are;
Hospice Georgina
Hospice Vaughan
Doane House Hospice
Evergreen Hospice
Hospice King-Aurora
Better Living offers a full basket of home and community care programs and services (i.e. meal delivery, personal and home support, adult day programs, exercise and falls, etc.) and as part of this integration the CLHIN has approved Better Living with funds to identify and procure a multi-tenant web-based software solution to align with current and future information management (IM) and information technology (IT) requirements that will support the operations of existing programs/services and the provision of central back office support of the five partnering hospice organizations for York Region.
REQUEST FOR PROPOSAL
Better Living is seeking to procure a single vendor to provide a multi-tenant web-based software solution that will provide the best overall value to Better Living and the five hospice organizations' programs and services for client records management, volunteer and human resource records management, reporting and data management, and back office support including, financial administration (i.e. accounts receivable and payroll). Qualified IM/IT vendors with prior experience are encouraged to submit proposals in response to this RFP.
