Quality Improvement Practice Facilitators
Practice facilitators to develop the capacity and expertise of patient centered medical homes to achieve systems transformation. Facilitators will coach primary care medical practices in implementing and managing process transformation including national recognition as patient centered medical homes; use of information technology systems such as registries and portals to improve patient care; integration of self-management support and planned care visits; redefining roles and establishing team-based care; and seamlessly connecting with community resources and specialty referrals.
