1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; this is a Request for Quotation (RFQ) and a written solicitation will not be issued. The solicitation number FA5209-17-U-0006 is issued using FAR Subpart 13.1 Simplified Acquisition Procedures.


2. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95 effective 19 Jan 2017, and the Defense Federal Acquisition Regulation Supplement (DFARS) Change Notice 20161222 effective 22 December, 2017, and the Air Force Federal Acquisition Regulation Supplement (AFFARS) Acquisition Circular 2017-0127 effective 27 January, 2017.


3. North American Industrial Classification Standard (NAICS) code is 333924 (Industrial Truck, Tractor, Trailer, and Stacker Machinery Manufacturing).


4. DESCRIPTION
Purchase of Two (2) Pallet Trucks for 374th Medical Support Squadron/SGSM at Yokota Air Base.
Minimum Equipment Specification Information is as follows:


o Equipment Description: Sugikoku 3LW3-50 Low-Lift Walkie Pallet Truck or equal.
o Load Capacity: Minimum 4300kg / 10000lb
o Fork Length: Minimum 2000mm
o Overall Width: Minimum 1200mm
o Individual Fork Width: Minimum 306mm
o Fork Range of Lift: 85mm (lowest) ~ 205mm (highest)
o Battery System: 24V, Minimum 164AH
o Input Voltage: 120V

6. Point of Delivery: Building 1586 at Yokota Air Base, Japan


7. Desired Delivery Period: 60 calendar days after date of contract.


The following contract provisions apply to this acquisition:

*If offerors fail to submit the followings, offerors are considered as non-responsive and ineligible for contract award.


(i) Company's complete mailing and remittance address.
(ii) Offerors shall complete the registration to have their own access for System for Award Management (https://www.sam.gov/portal/SAM/).
(iii) Quotations must be valid for a period of no less than 60 days.
(iv) Quotations shall contain following items:
(a) Price Schedule of Two (2) Pallet Trucks
(b) Technical Information
A technical description of the items being offered in sufficient detail to evaluate compliance with the paragraph 3.2 of Specifications (Attachment 1). This may include product literature (catalogue), or other documents, if necessary.
(c) Representations, Certifications, and Other Statements of Offeror or Quarters (Attachment 2)


Addenda to the following paragraphs of 52.212-1 are:


(a) (a) All quotes are due on or before 10:00 a.m. Japan Standard Time (JST), FRIDAY, 30 JUNE 2017. Electronic quotes are highly encouraged. Submission may be made via email to ikuyo.sakakibara.jp@us.af.mil and keisuke.kiyotaka.jp@us.af.mil, via facsimile at 042-553-7786/530-33319: ATTN: Ikuyo Sakakibara or Keisuke Kiyotaka. Hand carried or mailed proposals must be addressed to the address below (see part b of this addenda for additional information on paper submissions). Quotes must reference the solicitation number.

(b) For paper/hand carried submissions, the sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the solicitation number, and the name and address of the offeror. Offerors are cautioned that Yokota Air Base has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE EXPECTED WHEN HAND-CARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at Bldg. 620 prior to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.215-1(c) (3), "Submission, modification, revision, and withdrawal of proposals."


(c) All firms must be registered in the System for Award Management (SAM) database @ www.sam.gov
upon submission of request for quote (RFQ) to be considered responsive. If confirmation of SAM is not providing with the proposal submission by the closing date of the RFQ, the offer shall be considered nonresponsive and ineligible for contract award.

A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management (SAM) www.sam.gov. Include FAR 52.212-3 Alternate I, Offer Representation and Certification with Offer. No telephonic or facsimile responses will be processed.


8. Deadline for receipt of offers: 10:00 a.m. Japan Standard Time (JST), FRIDAY, 30 JUNE 2017.

9. Basis for Award
An award will be made to the lowest brand name product or equal IAW FAR 13.106-3 and FAR 11.104. The contracting officer determines that the proposed price is fair and reasonable. The Government will award a contract resulting from a solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
