The New Mexico Veterans Affairs Health Care System (NMVAHCS), located in Albuquerque, New Mexico is announcing the requirement for Base Year and Four Option Years Contract from businesses that are able to provide Contract Community Based Outpatient Clinical (CCBOC) Services. The Contractor shall provide continuous delivery of primary and preventive medical care services, including primary health services, for eligible Veterans enrolled with the NMVAHCS.
On behalf of the NMVAHCS, services for all assigned Veterans shall be performed at the Contractor's facility to be located within the city limits of Truth or Consequences (T or C), New Mexico. Contract will be for a Firm Fixed-Price.
This announcement is IAW FAR 5.203, as a combined synopsis and solicitation announcement ONLY; it is not a set-aside for Service Disabled Veteran Owned Small Businesses IAW FAR 19.14. Please complete the Request for Proposal (RFP), Standard Form 1449 and all other required documents and submit to this office not later than closing date on solicitation announcement. All Contractors shall be registered on WWW.SAM.gov, http://orca.bpn.gov and WWW.D&B.gov. The NAICS Code: 621498 (size standard of $19.0 million). Any amendments to this announcement will only be available through FEDBIZOPS website. Supporting evidence must be furnished in sufficient detail to demonstrate the ability and qualifications of the Contractor to provide this requirement.
