This is a Request for Proposals (REQUEST) issued on behalf of the Utah State Department of Human Services, Division of Child and Family Services (DHS/DCFS or Requesting Agency). The purpose of the REQUEST is to enter into a contract with a qualified Offeror for a service contract to provide adoption evaluation (commonly called home study) services for DHS/DCFS to evaluate if prospective adoptive families qualify to adopt children in the custody of DHS/DCFS.
