Print, bind and deliver Traffic Collision Report Forms.
BINDING: Overall 8 , width, ll length with 25 four part sets per book with a detached size of 8 X 11. One piece wrap around cover, printed on one side to be wrapped around open end of book. Each book shall have chipboard backing attached after last form.
FORMS: 4-part 8 x 11 inch detached plus a stub at the top.
Part 1: White 20# NCR CB black print paper
Part 2: Green 14# NCR CFB black print paper
Part 3: Green 14# NCR CFB black print paper
Part 4: Green 14# NCR CF black print paper
a.On Parts 2, 3 and 4 face printing only. The printing on parts in certain sections must register plus or minus 1/32" from part to part.
b.At the lower left hand bottom of Parts 2, 3 and 4 Face printing sheets, print in capital letters, DRIVER/OWNER, in red ink.
