This is a Sources Sought notice for the purpose of conducting market research. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement.
URGENT NEED: Further, this Sources Sought Notice seeks to identify a possible Service Disabled/Veteran Owned Small Business (SDVOSB or VOSB) source(s) for an emergency contract for a period of up to one-year (with a possible extension of up to six months, not to exceed a total contract term of 18 months). If a SDVOSB or VOSB has an immediately available qualified candidate(s) the Government shall consider making an emergency award(s) to a company if a services can be obtained at a fair and reasonable rate.
Background: The Mountain Home Veterans Affairs Medical Center (Mt. Home VAMC) is Primary Care Physicians (PCP) services for Veterans enrolled with the Mt. Home VAMC, including all associated Community Based Outpatient Clinics (CBOCs). Contractor would be responsible to schedule and provide services on an as needed basis. Please see attached for a better description of services the Contractor would be required to provide. Contractor would be expected to see approximately 1,000 to 1,200 Veteran patients annually.
Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method.
NAICS code: 621111 and the size standard is $12,000,000.
NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/; and must have an active System for Award Management (SAM) registration, with no exclusions.
Anticipated period of performance: A base period of one-year, plus four (4) one-year options (if applicable).
Other important considerations: All services are to be furnished at the Mt. Home VAMC, or one of the VAMC s associated CBOCs.B
Capability statement /information sought: Response to this announcement shall include the following information: (1) A tailored capability statement indicating the firm s ability to provide primary care physicians; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) candidate package for each immediately available PCP including CV, references, copy of unrestricted license, and DATE PCP IS AVAILABLE TO START WORK; and (4) Information should be provided electronically in a Microsoft Word or Adobe PDF format. Provide any other information that may be helpful in developing or finalizing the acquisition requirements.
