County intends to fund all of the following strategies/programs, and to make funding
available throughout the County. All programs should serve transition age youth at risk
of or experiencing the onset of serious psychiatric issues. Programs should also address
existing disparities in access to mental health services throughout the county, as well as stigma and discrimination faced by transition age youth at risk of or experiencing mental
health issues. All Applicants should address how they will participate in a system wide
integration of transition age youth mental health services across the prevention and early
intervention continuum, and strategies to link into ongoing publicly and privately funded
intervention and treatment systems of care. Funded Applicants will be required to participate
in a Connected by 25 Workgroup and other community efforts to serve transition age youth.
