This Request for Proposals is designed to generate proposals for a preferred lab card benefit option to integrate into Plans A and B for State Employees and their family members enrolled in the health plans, with an option for a discount arrangement for lab services for members enrolled in Plan C, the State’s Qualified High Deductible Health Plan option.
