RFQ 75H70120Q00001 - Stafford Septic System Replacement
Quotes are being requested under Request for Quotation (RFQ) no. 75H70120Q00001. This RFQ is prepared in accordance with FAR parts 13 and 36 as supplemented with additional information included in this notice. The Indian Health Service (IHS) intends to award a firm fixed price (FFP) construction contract using Simplified Acquisition Procedures. See RFQ and Attachments for all details.
This project is for the replacement of an individual septic system on Fee land in Pierce County, WA. All electrical work and backfill to be included in costs. Contractor responsible for utility locates and necessary permits. Contractor must be a licensed septic installer in Pierce County, WA.
This project will be set aside 100% for Small Businesses under North American Industry Classification System (NAICS) Code 237110 - Water and Sewer Line and Related Structures Construction with a size standard of $39.5 million.
The period of performance will be 30 calendar days after issuance of the Notice to Proceed.
Per FAR 36.204, the estimated price range of the resultant construction project is between $25,000 and $100,000.

Quote packages are due by October 15, 2019 at 2:00pm PST. Late quotes will not be considered. Quote packages SHALL be EMAILED to toby.hayden@ihs.gov and shall contain the information required by RFQ Section L, paragraph L.3. QUOTATION SUBMISSION REQUIREMENTS
The quote shall include the following:
1. Cover page to include:
- The solicitation number and acknowledgement of any solicitation amendments issued;
- The quoter's name, address, DUNS number, CAGE code;
- The name, title, email, phone number, and signature of the individual authorized to sign the quote; and
- A copy of the offeror's valid Washington State/County Plumbing License and/or On-Site Septic Installer Certification (Prime or Subcontractor)
2. Price Quote Submission
- SF1442, blocks 14-20c;
- Any and all solicitation amendments require acknowledgement in Block 19 of the SF1442; and
- SECTION B, Price Schedule, shall be filled out in its entirety (See Section J, Attachment J01).
In accordance with FAR 13.106-2, the evaluation for this award will be based on price and the quoter's compliance with the terms and conditions for this request for quotes; see Section L, paragraph L.3. Price is the discriminating factor for award among offers.
Questions/clarifications regarding this solicitation must be submitted in writing, via email, to toby.hayden@ihs.gov no later than October 10, 2019. Questions will be answered via amendment to the solicitation.
