Combined solicitation/synopsis NOTICE TO CONTRACTOR: This is a COMBINED SOLICITATION/SYNOPSIS for commercial service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13. The proposed contract action is to award to a vendor who can provide medical gases, delivery, pickup, and rental services for the Phoenix Indian Medical Center (PIMC) located in Phoenix Arizona. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number RFQ-18-PHX-01 is issued as a Request for Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95, dated 19 January 2017. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at this address: http://farsite.hill.af.mil/VFFARa.htm. The North American Industry Classification System (NAICS) Code for this acquisition is 325120; Size: # of employees 1,000. SERVICE ADDRESS: Division of Acquisition Management, Phoenix Area Office, Two Renaissance Square, 40 North Central Avenue, Phoenix, AZ 85004-4450. The Government will award a firm, fixed price contract resulting from this combined synopsis/solicitation to the responsive and responsible offeror whose conforming response is determined to provide the best value to the Government, price and other factors considered. The Government will only consider firm fixed-price quotations. Lowest Price Technically Acceptable (LPTA), using the following evaluation factors in descending order of importance: Factor 1: Technical; Factor 2: Past Performance; Factor 3: Price; the government shall conduct a price evaluation of all technically acceptable offers. Contractor must be registered to the System for Award Management (SAM) prior to award. A nine digit Data Universal Numbering System (DUNS) number provided by Dun and Bradstreet is required to register. The website address for SAM registration is https://www.sam.gov. Electronic submission of Quotes: Quotations shall be submitted electronically by email to Jeremy.Steel@ihs.gov. Email submissions are limited to 2MB. The submitter should confirm receipt of email submissions. The technical/past performance shall be separate from the price quote and is limited to 10 pages at 12 font. For your price quote please use the attached price schedule. Attachment 1 contains a Scope of Work. Attachment 2 contains a Price Schedule. Attachment 3 contains Wage Determinations. Quotations are due Tuesday October 3, 2017, 5PM EST.
