In accordance with Indiana statute, including IC 5-22-9, the Indiana Department of Administration (IDOA), acting on behalf of the Family and Social Services Agency (FSSA), requires provider or providers for pharmacy management services including one (1) full time Director of Pharmacy with valid Indiana License, one (1) full time Clinical Pharmacist with valid Indiana License and a Pharm D education and three (3) full time Pharmacy Technicians for Larue D. Carter Memorial Hospital (LCMH).
