Permanent Supportive Housing Program
The Louisiana Department of Health and Hospitals, Office of Aging and Adult Services is issuing a Request for Proposals (RFP) for qualified proposers to provide evidenced‐based individualized supports and services to people with disabilities, including homeless individuals with disabilities, who qualify for the State of Louisiana/DHH Permanent Supportive Housing Program in Orleans, St. Bernard, Plaquemines Parishes. The program is funded through the Community Development Block Grant (CDBG). The Contractor shall abide by the federal and state regulations governing these funds. A Pre‐Proposal Conference to obtain RFP clarifications will be held December 18, 2008 at 1:00 pm CDT at the Region I Office of Aging and Adult Services, 1010 Common 2nd floor, New Orleans, LA. Attendance is optional.
