The AAFP, AAP, ACP and AOA have defined the Patient-Centered Medical Home (PCMH) as a model of care in which each patient has an ongoing relationship with a personal physician who leads a team that takes collective responsibility for patient care. The physician-led care team is responsible for all the patients health care needs and, when needed, coordinates care across the health care system. We are committed to the PCMH model of care and have developed a strategy that highlights our role in helping practices transform into medical homes:
1. Education
2. Operational support
3. IT support
4. Reimbursement changes
In November 2008, we began supporting five PCMH pilot programs and are now accepting applications for 8-10 Phase II pilots. Through these pilots, we are committed to:
1. Alignment of goals among patients, providers and plan
2. Infrastructure changes in primary care that are sustainable and support comprehensive, evidence-based care for all patients
3. Affordability by strongly supporting both the service mission and the fiduciary responsibilities of caring for our communities
