This combined synopsis/solicitation, W911YP-18-R-0004, and is being issued as a small business set-aside Request for Quotation (RFQ). Northern America Industry Classification System Code (NAICS) is 713940, Fitness and Recreation Sports Centers; NAICS code 811490, Sporting equipment repair and maintenance without retailing new sports equipment is also acceptable.
The Utah Army National Guard has a requirement for monthly preventative maintenance on UTNG fitness equipment and on-call repair and part reimbursement. See the attached performance work statement and list of machines for details. The contract type for this procurement will a combination Firm Fixed Price (FFP) and Time and Material (T&M). CLIN 0001 will be a base plus four option year FFP CLIN for the monthly preventative maintenance, and CLIN 002 will be a base plus four option year T&M for the on-call repair and part reimbursement.
Evaluations Factors: Award will be made on the basis of best value. Price will be the primary consideration; however, firms or individuals submitting quotes must be technically qualified, holding the necessary certificates for the equipment subject to this solicitation, to perform the required work. Interested vendors are instructed to provide evidence of qualification. The Government reserves the right to award to other than lowest quote if that low quote does not demonstrate the ability/qualifications to meet the requirement.

All firms or individuals responding must meet all standards required to conduct business with the Government, including registration with SAM and WAWF/iRAPT, and be in good standing. All qualified responses will be considered by the Government. Quotes must include CAGE/DUNS number, federal tax number and company's point of contact information. Please provide your submission to the United States Property and Fiscal Office Purchasing and Contracting Division no later than 9am (Mountain) 18 June 2018. All submissions should be sent via email to: otha.b.henderson.mil@mail.mil. Facsimiles are not acceptable. Question regarding this requirement must be submitted via email, with sufficient time to do any research to answer question prior to the closing of this posting. Responses received after the stated deadline will be considered non-responsive and will not be considered.


In accordance with FAR 52.212-1(k), prospective awardees shall be registered and active in the System for Award Management (SAM) prior to award and be in good standing. Offers may obtain information on registration at www.sam.gov . Failure to provide SAM registration may result in your quote being disqualified from consideration.


This combined synopsis/solicitation incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The full text of a clause may be accessed electronically at http://farsite.hill.af.mil/ .


IAW FAR 52.204-7 and DFARS 252.204-7004 Alt A, prospective vendors must be registered in the SAM database prior to award. Lack of SAM registration shall be a determining factor for award. Prospective vendors should visit the SAM website at http://www.sam.gov/start.aspx to register.


Instructions to interested vendors: Site visits will be conducted on 30 May 2018 for interested vendors to view our equipment in order to better provide an accurate quote. Only quotes from those vendors who attend a site visit will be considered. There may be one additional site visit between 30 May and the close of this solicitation (time and date TBD) if there is a need.
