PRESCOTT FIRE CENTER REFLOORING
This is a combined synopsis/solicitation for commercial items prepares in accordance with the format in Subpart 12.603(c)(2)(i), as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested. This solicitation AG-8191-S-14-0014 and statement that the solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular.
The provisions in 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition and FOB point.
The provisions in 52.212-2, Evaluation - Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, described the evaluation procedures to be used.
In povision in 52.213-3 Offeror Representation and Certifications - Commercial Items, shall include a completed copy.
The provision in 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition.
The provision 52.212-5 Contract Terms and Conditions Required to Implement Statues of Executive Orders - Commercial Items, applies to this acquisition. If any, of the additional FAR clauses cited in the clauses are applicable to this acquisition.
Any additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements determined by the Contracting Officer to be necessary for this acquisition and consistent with customary commercial practices.
USDA Forest Service - Prescott National Forest has a reflooring project at the Prescott Fire Center. This project consists of removing and replacing approximately 5,100 square feet of carpet and 3,700 square feet of tile at the Prescott Fire Center. This is to include any new underlayments, adhesives, transitions, cove base, and disposal of old carpet/tile and all associated items.
