COMBINED SYNOPSIS/SOLICITATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice.
This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-95, dated January 19, 2017.
Solicitation VA261-17-Q-0558 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract.
This procurement will be issued as a Service Disabled Veteran Owned Small Business Set-Aside. NAICS Code is 238190 and the Small Business Administration s (SBA) size standard 1000 Employees.
The Government is seeking a contract to acquire wheelchair ramp. The wheelchair ramp is used to provide veterans a better quality of life. This solicitation is for a ramp that is ADA Compliant. Offerors shall only submit ADA compliant quotes.

Period of Performance: Within 15 days of placing order or sooner if available.
Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process.
Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm s DUNS#, total price. Offerors shall have completed FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items in Systems for Award Management SAM at www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items.
Submit all question(s) to Bernie Byrd, Bernie.byrd@va.gov & Lelani Basuel, lelani.basuel@va.gov no later than May 31, 2017, 4:00 PM Pacific Standard Time (PST). Ensure to reference solicitation # VA261-17-Q-0558 within subject line of the email. Contact the above for specific patient address.
Quotes must be received by June 2nd 2017, 4:00 PM. Email your quote to Bernie Byrd, Bernie.byrd@va.gov & Lelani Basuel, lelani.basuel@va.gov. Ensure to reference solicitation # VA261-17-Q-0558 within subject line of the email. Quote(s)/offer(s) received after the prescribed deadline, shall be considered non-responsive and will not be considered for award.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and is the Lowest Price Technically Acceptable (LPTA).

The Government intends to evaluate quotes and award a contract without discussions (except clarifications). Therefore, the offeror s initial quote should contain the offeror s best terms from a price and technical standpoint. Award will be made to the responsive, responsible, lowest priced, technically acceptable offeror that meets or exceeds the acceptability standards for all non-cost factors.

The following factors shall be used to evaluate offers:

SDVOSB Verification- The Government will review VetBiz (https://www.vip.vetbiz.gov/ ) to confirm Service Disabled Veteran-owned Small Business (SDVOSB) status verification for the submitted quotes. Evaluations will be performed on quotes submitted by verified SDVOSBs only.
Evaluation Overview: Quotes will be evaluated and ranked according to price, from lowest to highest. The Government reserves the right to evaluate only the technical quote of the lowest priced offeror. Other technical quotes may not be evaluated, unless the Government determines the lowest price offeror to be unacceptable or non-responsive. In the event the Government determines the lowest price offeror to be unacceptable, the technical quote of the next lowest price offeror will be evaluated. This process will continue until a technical quote is determined to be acceptable.
Offers will be evaluated utilizing the following Technical evaluation criteria:
3.1 Meets ADA Compliance: The ADA Compliant element will be rated either acceptable or unacceptable. This element must be rated acceptable to receive an overall acceptable technical rating.
Past Performance- The Government will utilize information found in the Past Performance Information Retrieval System (PPIRS), Excluded Parties List System, System for Award Management, Experian, and Contractor Performance Assessment Reporting System.
Price - Price will be evaluated and ranked according to price, from lowest to highest. Price will not be evaluated other than to ensure price is fair and reasonable.
