PILOT PRIMARY CARE CLINIC MODELS OF CARE DELIVERY FOCUSED ON CARE COORDINATION AND PATIENT/FAMILY INVOLVEMENT
The Department is seeking Proposals from qualified Responders for Care Coordination Pilot Projects. DHS will award grants to support Pilot Projects for children and adults with complex health care needs who are enrolled in fee-for-service Medical Assistance (MA) program.
Qualified responders include collaboratives and organizations that support multiple clinics. Providers and clinics that are interested in practice transformation and certification as a health care home are invited to apply as well.
