PHYSICIAN/MEDICAL AND ON-CALL SERVICES

The delivery of services to be provided to individuals living in community-based residences is
expected to be developed over the next year and will include medical evaluations and care for
those for whom medical services cannot be accessed; either because of their behavioral/ medical
complexity or other reasons. Integration of medical services with those services that can be
accessed in the community will be emphasized. It is expected that these services will be mostly
consultative. This program will be administered within the office of the Executive Director. Examples
of medically-related services that will be rendered as part of this program might include:
1. Adult neuro-developmental assessments,
2. Primary care checks; including chronic disease management,
3. Oral health assessments,
4. Health prevention and promotion assessments, including fall assessments,
dementia screening, vision and hearing checks,
5. Integrated primary care psychiatric services,
6. Enterostomal and skin evaluations,
7. Medication reviews.
