Outreach & Informing Activities
The Commission issues this Notice of Enrollment (the Notice) requesting applications from hospitals with a high number of emergency department non-acute care Medicaid claims to utilize Promotores(as)/Community Health Workers (P/CHWs) to provide outreach and informing to clients and/or caretakers of Medicaid recipients birth through age 20 (Texas Health Steps recipients) about Texas Health Steps benefits and services, assist them with identifying a medical and/or dental home, linking the child with needed medical/dental services, such as transportation, and follow-up to encourage the child gets periodic checkups.
