Optometry Services for Southeast Health Center (RFQ 8-2010)
The Department of Public Health, Community Primary Care, is issuing RFQ in search of candidates from Accreditation Council on Optometry Education (ACOE) accredited educational institutions to provide .1FTE of Optometrist to provide vision care services for patients at one of the community Primary Care sites-Southeast Health Center. The services will include on-site eye exams, preventive eye care services, make diagnostic referrals, and provide all necessary equipment to provide full scope eye care services for the period of July 1, 2010 through June 30, 2011. The contract shall have an original term of one (1) year. In addition, the City shall have four (4) options to extend the term for a period of one (1) year each for a total of a five (5) year contract, which the City may exercise in its sole, absolute discretion. The maximum amount available for the Fiscal Year 2010-11 is $25,000, which may increase or decrease subject to the funding availability and clinical need for services.
