This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

This solicitation 36C25019Q1072 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-02/06-05-2018. The North American Industry Classification System (NAICS) number is 339112 and the business size standard is 1000 employees.

BRAND NAME ONLY REQUIREMENT:

It is imperative that your quote include all necessary items to outfit a new ophthalmologic exam lane to include:

Reliance exam chair - #206 charcoal
Reliance 7900-IC floor stand with instrument console
Reliance pneumatic stool - #206 charcoal
Haag-Streit 900 BQ LED slit lamp
Haag-Streit BP/BQ/BX tonometer
Marco CP-770 automatic wall mounted chart projector
Reichert 11625B refractor
Keeler Vantage Plus LED wireless indirect with subline charging system and front surface mirror set

NOTE: This equipment will need to be delivered to the eye clinic and requires professional installation by an authorized technician. This eye exam lane will match 17 existing exam lanes currently in use by VA staff who are trained on this equipment. The VA biomed department is tasked with repair and routine maintenance. This BRAND NAME ONLY REQUIREMENT streamlines training and repair and saves VA funds in the process.

The proposed contract listed here is SET-ASIDE for Service Disabled Veteran Owned Small Businesses (SDVOSB s) only. FOB destination.
Deliver to VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105.
