The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research. This Sources Sought is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government at this time. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine interest and capability of potential vendors, including Small Businesses, Veteran-Owned Small Business (VOSB), and Service-Disabled Veteran Owned-Small Business (SDVOSB) vendors relative to the North American Industry Classification System (NAICS code) 339115 (Ophthalmic Goods Manufacturing).
The Illiana Health Care System specifically the VA Decatur CBOC-- has a need for 2 complete working Ophthalmology units. The Decatur CBOC needs these units, as the construction currently underway involves 2 separate Ophthalmology rooms for addressing Veteran Patients Ophthalmology needs. The Decatur CBOC is requesting Brand Name Only, as these salient characteristics and each unique part herein works together and allows limited mobility patients full access to Ophthalmology Care.

****Gray Market Prevention Language****
** (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.B No remanufactures or gray market items will be acceptable.

(b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.B All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.
(c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.B Accordingly, the B VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to B replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.B

Your response will be used solely for market research purposes of the Government. The Government will not pay for any materials provided in response to the notice and submittals will not be returned to the sender.
