Operate an Individual Assessment Center

The Department is seeking a Contractor to operate an Individual Assessment Center. The Center will serve as a point of entry for homeless single adults and will provide: assessment and eligibility determinations; prevention/diversion resources; and, shelter and housing placement referrals for homeless single adults seeking shelter. The Department is seeking a qualified Contractor to operate the Center to reflect the system redesign for the homeless services Continuum of Care.
