Occupational Health Services
The purpose of this Request for Proposal is to establish an annual contract with qualified firms to provide professional services to be rendered by a physician for City applicants and employees. The City anticipates approximately one hundred (100) pre-employment physicals, fifteen (15) disability retirement physicals, forty (40) fitness for duty evaluations, forty (40) DOT Physicals, and ten (10) Medical Records Reviews to be administered annually.
City Physician must be licensed to practice medicine in the State of Georgia.
