Nursing Assessments/Reassessments
County Department of Social Services ("WCDSS") Office of Medical Home Care Services is requesting proposals from all public, not-for-profit, or proprietary agencies wishing to provide nursing assessment/reassessments to Social Service customers who need Personal Care Services, establish a plan of care for the customers and provide supervision of the customers to determine if the level, amount, frequency and duration of Personal Care Services authorized continues to be appropriate, as specified in 18 NYCRR 505.14 of Social Services Regulations. Proposers, if chosen, will be required to provide nine (9) Registered Nurses (or equivalent of 9 full time nurses) and adequate Nursing Supervision by Registered Nursing Supervisors. We are requesting an explanation of how full coverage will be maintained even when some nurses may be unavailable such as due to illness or vacation.
