Notice of Open Enrollment for Medical Home Supports The Department issues this Notice of Open Enrollment (the Notice) requesting applications from the following types of primary care practices (Note: subspecialist practices attesting to be a medical home as defined in this open enrollment may also apply): 1) Pediatric practices serving children and youth up to age 21, including children and youth with special health care needs; 2) Family physician practices serving children, youth, and young adults up to age 25, including children, youth, and young adults with special health care needs; and 3) Internal medicine practices serving young adults from 21 to 25 years of age, including young adults with special health care needs. A practice may apply to be a Contractor for Medical Home Supports to implement practice-level supports to improve the provision of a Medical Home for children and youth with special health care needs (CYSHCN) (birth to age 21) and/or to facilitate effective transition of young adults with special health care needs to a Medical Home within the adult health care system.
