North Cheyenne Health Center Flat Roof Replacement
This is a Sources Sought notice. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. This Sources Sought is a market research tool being utilized to determine the availability of qualified small business concerns prior to issuing a Request for Proposal. This is not a solicitation for proposals and no contract will be awarded from this announcement.
No reimbursement will be made for any costs associated with providing information in response to this announcement. All responses will be provided by an e-mail response. No presentations will be scheduled.
In accordance with the Federal Acquisition Regulation 5.2, Indian Health Services is conducting a Sources Sought notice for the removal of an existing roof and the construction of a flat roof at one of its healthcare facilities.
This service will take place at the North Cheyenne Health Center, Lame Deer, MT, 59043.
The Indian Health Service (IHS) seeks proposals for a construction contract that will provide the complete roof replacement at the North Cheyenne Health Center. The current roof system is approximately 52,200 SF. The existing flat roof is a smooth surface built-up roof with tapered insulation. IHS has a fully designed single ply membrane roof with tapered insulation.
The removal of existing rooftop equipment is required to be stored for re-installation once the new roof has been constructed and installed. Removal includes the existing roofing and flashing system down to the metal deck level. Once removed, the new roofing and flashing system shall be constructed and installed in accordance to the provided design and shall incorporate the re-installation of the previous rooftop equipment. All necessary testing for roofing and equipment is required for a complete and usable roof at the facility.

2) TERO fees or Tribal Taxes
Interested small businesses are invited to respond to this sources sought by providing the below information.
The written response to this notice should consist of the following items:
a. Company Name.
b. Company DUNS number.
c. Company point of contact, mailing address, telephone and fax numbers, and website address
d. Name, telephone number, and e-mail address of a company point of contact who has the authority and knowledge to clarify responses with a Government representative.
e. Date submitted.
f. Type of Company: All respondents should register on the SAM located at https://www.sam.gov/portal/SAM/#1.
g. Provide 3 sample service awards amount ranging from $500,000.00 to 1,500,000.00. On each of the sample state the following:
1. Was your firm the prime?
2. What percentage of the work did your firm complete?
3. What type of service was it?
Sample projects completed within the past five years are considered more relevant than projects completed more than five years ago. Sample projects with federal, state, or local Government, and/or commercial customers will be accepted.
Respondents will NOT be notified individually of the results of the sources sought.
Important Notice:
It should be clearly understood that this Sources Sought notice is not an award or commitment by the Government. The offeror is further advised that funding may not become available. If funds are not available, no award will be made. The offeror will not be reimbursed for any/all effort or submission preparation costs.
