New LIM (Isolated Power Panel) Install
Dear Ladies and Gentlemen:
Reference: 14-247-SOL-00088 - Seeking vendors that can successfully provide, fabricate, retrofit, install and commission a new LIM (Isolated Power Panel) at the Phoenix Indian Medical Center (PIMC) Operating Room#5.
You are invited to submit a Request for Quote (RFQ) to the referenced solicitation. The following documents must be submitted in response to this solicitation as follows:
1. Standard Form (SF) 1449, Solicitation/Contract/Order for Commercial Items, requires completion of section nos. 12, 17, 23, 24, & 30.
2. Price Quote must be submitted in Accordance to Paragraph B, Pricing.
3. Liability Insurance Certificate on Page 7.
4. Response to Evaluation Factors on Page 17.
5. Check Boxes on Page 20 under 52.212-3 -- Offeror Representations and Certifications.
6. Tax Payer Identification Information on Page 30.
All sections must be completed otherwise the IHS will not consider your RFQ.
Complete proposal are due by 12:00 P.M., Phoenix, Arizona Time on July 25, 2014.
