This is a Subcontract Opportunity with Education Management Corporation, not United States Department of Labor.  Phoenix Job Corps Center is currently soliciting bids for a Blanket Purchase Agreement (BPA) to provide Ditty Bags for approximately 192 males student and 161 females students a total of approximately 276 male ditty bags  and 232 ditty bags per year.

1) Vendor must submit the prices on the attached form
2) Vendor must be registered with sam.gov
3) Vendor must submit company W-9
4) Vendor must complete the following vendor information forms:
     a)  Vendor Questionnaire, Vendor's Acknowledgement Form
           and Conflict of Interest Form
