Sources Sought DENTAL LAB SERVICES

The Department of Veterans Affairs in Altoona, Pennsylvania requires a contract for dental services for patients at the Altoona VA Medical Center (AltVAMC), which is located at 2907 Pleasant Valley Blvd. Altoona, PA 16602. The Contractor shall provide all labor, supervision and all other resources required to deliver dental lab services to include work by qualified dental lab personnel, materials, repairs, and finished products, for and pertaining to the items listed in the schedule, in accordance with the contract documents. The contracted dental lab will fulfill the standards set by our current responsiveness to the Veterans of the Altoona VAMC population as determined by current standards of customer satisfaction or established wait times for dental products. The contractor must be capable of meeting and maintaining the Turn-Around Times (TAT) stated in the solicitation.

The Altoona VAMC intends to award a five year contract for these services.

STATEMENT OF WORK DENTAL LAB SERVICES


Dental Laboratory Services
Fixed and Removable Prosthetics

A. General Requirement

The Department of Veterans Affairs Medical Center, Dental Laboratory, Altoona, PA seeks to contract with a dental laboratory to provide all steps in the fabrication of fixed and removable prosthetics in support of our veteran population. This contract will be for a base period for one year from date of award, plus four (4) option years.

B. Specific Requirements

The Contractor shall use procedures, techniques and current technology that are considered to be the standards in the industry. The Contractor shall provide items that meet the esthetic and functional requirements of the patient. The Contractor shall be familiar with mold guides by Trubyte Bioblend and with shade guides by Trubyte Bioform and the VITA 3D Tooth guide. High quality materials shall be utilized from ISO 9001/CE registered companies. Laboratory shall perform internal technical audits to ensure consistent, high quality, and timely restorations.

The Contractor shall provide the following dental laboratory services to the VA Medical Center, Altoona, Pennsylvania:

Preparation of Models (pouring impressions, trimming, study, final).
Mounting models from provided impressions and articulation of cases.
Fabrication of custom trays (rigid with handles).
Fabrication of base plates with wax occlusal rims for full and partial dentures.
Set up of denture teeth for full and partial dentures (including one reset as needed).
Processing and finishing in acrylic of full and partial dentures (including placement of ID tag).
Repair of full and partial dentures (reline, rebase, replace tooth, addition of tooth or clasp).
Reline options offered including PermaSoft or soft reline equivalent.
Design, construction, and finishing of metal frameworks for cast partial dentures.
Fabrication of acrylic partial dentures with wrought wire clasps.
Construction of diagnostic wax-up models.
Fabrication of interim maxillary dentures.
Fabrication of dental implant supported full and partial dentures (including locator attachments).
Provision of blank prescription forms to be completed by VA dentist.
On-site consultation of cases as needed, contractor will not have access to patient information.
Fabrication of orthotic appliances and soft occlusal guards.
Fabrication of surgical stents for implant placement (templates).
Fabrication of porcelain fused to metal crown/bridge, full contour Zirconia crown/bridge, EMax ceramic crown/bridge, and surveyed crowns.
Construction of Maryland bridge appliances.
Fabrication of Fixed Partial Denture Bridge (multiple units restorations).
Fabrication of Implant abutments, custom abutments, and implant crowns (screw and cement retained).
In addition to the listed services, the following requirements are to be provided by the Contractor for such services:
The Contractor must identify the specific type of metal used in each prosthesis at the time of delivery. The Contractor shall provide metal alloy manufactured in the United States of America to fabricate all required services. The contract laboratory shall identify by affixing the most recent manufacture label to provide a description of the metal alloy components and percentages used in the fabrication of each of the requested dental prostheses.
The Contractor shall provide a thorough infection control process for all incoming and outgoing cases. All completed cases must be disinfected before the product is returned to the VAMC Altoona.
The Contractor shall provide daily personalized pickup and delivery service to ensure that casework arrives on time and safely and no additional cost to the VAMC Altoona.
The Contractor shall provide local reline service with same day turnaround of all dentures (relines and rebases) as needed.
The Contractor shall provide same day repair service of all dentures and partials as needed.
The Contractor shall provide on-site, same day professional case consultation at no additional cost to the Altoona VAMC.
The Contractor shall provide same day all ceramic crown service as needed.
The Contractor s Officer Representative (COR) and the Contracting Officer (CO) shall be notified if the contracted laboratory intends to subcontract any work under this contract to another dental laboratory. The subcontractor must be located in the United States of America and the Contracting Officer must provide approval to Contractor to subcontract services.
The attached Quality Assessment of Dental Laboratory Procedures will be used to measure/assess the quality of the product produced.
