1. DESCRIPTION OF SOLICITATION

In accordance with FAR Part 13 this a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

This Solicitation is issued as a Request for Quotation (RFQ) for the following Commercial Items:

General Equipment Name: Natus Optometrics Oculomotor Package
Description & Salient Characteristics: The Optometrics Oculomotor Suite of Tests is designed for testing vestibular (balance) function with reflex eye movement responses. We want to use this lightweight, portable device to assess vestibular dysfunction in Veterans with Traumatic Brain Injury (TBI) and also eye movement accuracy (saccade accuracy). We have ongoing prospective research to assess manifestations of TBI in Veterans and also to assess changes over time and in response to vestibular therapy. This video-based goggle device would be ideal for outpatient testing of Veterans, especially with those that have vestibular and eye movement symptoms from TBI, multiple sclerosis and stroke to the brainstem area.

Impulse Monocular Video Frenzel
Lightweight USB goggles with advanced modular software that allows recording of the eye with high-resolution image quality.
Modular, customizable balance assessment solution that gives more ways to help patients with vestibular disorders.
Record eye movement with vision or vision denied (eliminating fixation).
Room video records in grayscale or color.
Video frenzel goggles enable you to monitor the elapsed time with a recording indicator and start and stop recording using a presentation remote control.
You can also display the eye video on a separate external monitor and the laptop.
Ability to play back eye and room recordings synchronously. Play back at normal speed and slow motion.
Identify which video contains what information at a glance.
Add a remark or note to the video to help you identify the test being performed whether it is spontaneous nystagmus, gaze or positional.
Add an indicator to any video that can alert you if the data is unique or of particular interest.
Create customized reports, share and store data easily using Otosuite Vestibular reporting and proprietary Otosuite Vestibular database and data sharing.

Lateral Head Impulse
ICS Impulse is the only vHIT system validated in the horizontal planes and approved by head impulse testing industry pioneers, Drs. Halmagyi and Curthoys. It provides quick, clear-cut side of lesion specific assessment of the vestibule-ocular reflex response to stimuli in the high-frequency range, the natural range of head movements. It is the only test that can assess all six semicircular canals

LARP/RALP Head Impulse
ICS Impulse is the only vHIT system validated in the lateral planes and approved by Drs. Halmagyi and Curthoys.
It provides quick, clear-cut side of lesion specific assessment of the vestibuloocular reflex response to stimuli in the high-frequency range, the natural range of head movements. It is the only test that can assess all six semicircular canals

Monocular Oculomotor Limited
The oculomotor module from ICS Impulse delivers the oculomotor tests you need to determine central and peripheral disorders, including:
GazeB the ability to assess for both gaze-evoked nystagmus and spontaneous nystagmus. These tests can be performed with vision or with vision denied.
Visual VOR (Vestibulo-Ocular Reflex) and VOR Suppression (VORS) tests to identify the presence or absence of saccadic eye movement. The unique design of the goggles presents the suppression stimulus from the goggles in order to assess VORS in any location.
Skew deviationB assesses the patient s ocular alignment by identifying if ocular misalignment occurs because of covering and uncovering the eye.
Torsional Module
Torsional provides a more accurate diagnosis and monitoring of vestibular patients.
With ICS Impulse 3D Nystagmus Analysis:

4. EVALUATION OF OFFERS

The Provision FAR 52.212-2, Evaluation Commercial Items (Oct 2014) applies to this acquisition and the following two lines are to be used and inserted into paragraph (a) of the that provision:

LPTA The basis for award will be Lowest Price Technically Acceptable - evaluated on technical capability of the item offered to meet the Government requirement. The government will award a contract resulting from this solicitation to the responsible offeror who offers the Lowest Price Technically Acceptable quote. The government shall evaluate the lowest quote first and if found technically acceptable will not evaluate other quotes.

Past performance IAW 9.104-3(b). The government will consider past performance when making a contractor responsibility determination IAW 9.104-3(b).

Original Equipment/No Gray Market Goods

(a) This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured, used, expired, outdated (old edition or old model) or gray market items will be acceptable. Gray market items are Original Equipment Manufacturer (OEM) goods sold through unauthorized channels in direct competition with authorized distributors.

(b) Offeror shall be a distributor that is authorized directly by the manufacturer (OEM) of the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter from the manufacturer. The OEM s warranty and service shall be provided and maintained by the OEM and pass through to the VA. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions and shall pass through to the VA.

(c) The delivery of remanufactured, used, expired, outdated (old edition or old model) or gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
