The selected vendor will be responsible for the following:
1) Assessing nutritional needs of women and their families (e.g. infants,
children, partners) with the goal of improving the nutritional health of
the woman and family.
2) Developing therapeutic diet plans.
3) Teaching the effects of nutrition on health and birth outcomes.
4) Teaching how nutrition can reduce obesity and overweight.
5) Retaining adequate staff to provide services to women across the
state.
Nutritional counseling will be provided through the Northern and Southern
Health Services network of sites. Referrals to the selected nutrition vendor will
come from HWHB medical vendors who do not offer on-site nutrition services
and the DPH nurse-home visiting program. It is expected that approximately
1,000 women will be served per year.
The Healthy Women/Healthy Babies (HWHB) program is designed to provide
access to preconception care and enhanced prenatal care for women in
Delaware through a network of contracted medical vendors. HWHB focuses
on core clinical and mental health services including clinical interventions,
mental health counseling, prenatal care/interconception and nutrition.
Based on an initial risk assessment, women with a Body Mass Index greater
than 30 or those deemed at risk for nutritional deficiency will be referred for
nutritional counseling.
