GENERAL DESCRIPTION:


The Pine Ridge Hospital intends to award a Firm Fixed-Price, Non-Personal Service contract for Six (6) Adult Nationally certified Nurse Practitioner (NP), or Physician Assistant (PA) services at the Pine Ridge I H S Hospital, E. Hwy 18, Box1210, Pine Ridge, SD 57770. Period of Performance will be 11/04/2019 through 11/03/2024


The "Unit Price" is an all- inclusive cost. All "inclusive cost" is defined to include travel, lodging, per diem, fringe benefits, i.e., life insurance, social security, federal, state, and local taxes, plus all other costs pertinent to the performance of this purchase order/contract/DO/TO.


BASIS FOR AWARD: Quoters are advised that the Govenm1ent will use lowest price technically acceptable to determine the successful contractor. Quoter's are advised to provide a Certificate of Lia bility Insurance with their quote and the required government forms need to be completed and submitted with the bid packet to be considered a viable bid.

All required documents listed below must be completed, signed, and submitted in order to be considered responsive and considered for award.
1. SF 1449- Request for Quotation
2. Licensure Requirement
3. Security Clearance
4. Statement of Work
5. Contractor Forms, OF306, Waiver, licensure, Child Care. If these forms are not received in the bid packet then the packet is incomplete). These need to be completed by all staff that will be performing services on this award.


OIG Exclusion List - No contract award shall be made to any vendor or provider listed on OIG Exclusion List - http://exclusions.oig.hhs.gov throughout the duration of the contract. Should the candidate be found with a non-favorable security clearance it shall be the contractor's responsibility to replace the candidate working under this contract with a suitable candidate. It shall be the responsibility of the contractor to notify the Acquisition Office if there is a change in provider.


The selected offeror must make arrangements with the Service Unit Background Investigation Coordinator and complete the following as soon as possible:


- Fingerprints
- SF-85 Questionnaire for Non-Sensitive Positions (done electronically)
* When bidding be sure to check that your GSA/FSS covers South Dakota as a geographical location.
