NEPHROLOGY AND DIALYSIS SUPPLIES
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
The Regional Health Contracting Office - Pacific, Bldg 339 146 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, if necessary, and award an Indefinite Delivery Indefinite Quantity type contract with Firm Fixed-Prices for nephrology and dialysis supplies for the Dual Renal Replacement Therapy (RRF) and Therapeutic Plasma Exchange (TPE) NxStage model System One S with NxView. This is a request for proposal (RFP). This solicitation number is W81K0218R0004. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. The Standard Industrial Classification Code is 6515, size standard is 750 employees and the NAICS code of 339113. This procurement is being solicited as a Brand Name Only requirement.

The Government intends to award a Firm Fixed Priced contract for a base period of 01 October 2018 through September 30, 2019 with four (4) one-year options to the successful quoter.
Contract Requirements - The successful quoter shall be required to meet the requirements of the attachment; W81K0218R0004AttA. Contractors shall quote their unit pricing based on the maximum quantities identified in each item.
